We are open to the public from 8:30 a.m. to 4:30 p.m. Monday-Friday. Check for schedule changes around holidays. We are closed on major holidays.
Cost of a vital record:
The cost of $5.00 for a death certificate and $6.00 for a birth certificate. NO CHECKS PLEASE. Cash or Money Orders only payable to: Dearborn County Health Department. Bring the completed form to our office, or mail your form to our main address listed above. We will complete the search and send you the results.
Adoption and original birth information
Adoption record requests are available through the State of Indiana and you should contact the Indiana Department of Health, www.in.gov . For direct access, several State form links are shown in the right hand column as well.
Correcting information on a certificate:
There are several provisions in the Indiana statutes whereby changes may be made on a vital record. We encourage you to write us with your correction request and we will advise you on what needs to be done to make the correction.
