How do I get a copy of the death certificate?

Cost: death certificate $10, NO CHECKS PLEASE. Cash or Money Orders payable to: Health Department. Bring the completed form to our office, or mail your form to our main address listed. We will complete the search and mail you the results.

Certified copies of birth and death certificates are available from the Indiana State Department of Health (ISDH) on-line with payment using a credit card through the Vital Records Online Ordering System. If you prefer to pay by check or money order, please submit the request via mail.

You may contact the local health department in the county where the event occurred to obtain information on their walk up service hours, method of acceptable payment and if they accept order via the internet. All requests require proper identification and proof of relationship to the person whose record is being requested is required. Methods of payment and fees may vary.
