Vital Records

The Vital Records section of the Health Department maintains and issues birth and death certificates occurring in Dearborn County, Indiana.

Our records begin in 1882, and the open record policy is also available for Genealogy research, based upon the 75 year viewing rule. The State Department of Health Vital Records begin in 1907.

Dearborn County Vital Record Certificate Costs:

  • Death certificate             $20.00 Short form   $30.00 Long form
  • Birth certificate               $15.00 
  • Paternity Affidavit           $50.00
  • Amendments/Corrections (with new birth certificate issuance)  $25.00
  • Genealogy (per record)    $10.00 Please see application for explanation of costs.

A photo ID with the star is required to obtain certified records for self. To obtain a record for someone else, proper documents to show proof of relationship is required. Request forms can be found below in the "News" tab.

Credit Cards are accepted with an added $1 administration fee during the transaction. Cash, Money Orders and Cashier's Checks are accepted, however, personal checks are not accepted.

In some instances, such as settling an estate, the 'Long form death certificate' may be required by that agency, for further questions regarding the long form, please call our office.

Records may be requested by mail, please go to the appropriate link below in the "news" section of Health Department.  Print the request form, complete, mail in with payment and include proper documentation to show relationship. If you have any questions, please call:
